Certificate of Origin Forms – in order to ship anything overseas, the shipment must contain a Certificate of Origin. These documents confirm the items shipped came from the place claimed. Chambers of Commerce have the authority to approve a Certificate of Origin. When you are ready to ship, simply complete the attached form and contact your local Chamber:
